Eligibility and Forms

Required Forms

To participate in a school sport, students must meet academic eligibility requirements, which include having passing grades in four out of their five classes from the previous semester.

To try out for a team or attend practice, students must have the following forms submitted to their coach:

1. Athletic Physical Form
2. Insurance Waiver
3. Concussion Awareness Form
4. Cardiac Arrest Awareness Form
